Lactational Amenorrhea Method
The Lactational Amenorrhea Method (LAM) of family planning takes advantage of the natural decline in fertility provided by breastfeeding. However, practicing breastfeeding for birth control is different than general breastfeeding. Specific guidelines are required to achieve the 98% or higher effectiveness rate that is possible with this method.6-9
Three conditions must be met to ensure the highest efficacy of LAM.6,8-10 One, the woman must "fully" or "nearly fully" breastfeed her infant (often referred to as "exclusive" or "complete" breastfeeding). Two, the woman must remain amenorrheic. Three, the method is most effective during the first 6 months postpartum. Nipple stimulation is essential for effectiveness, which means the mother must be feeding frequently (including nighttime) for the method to work. Regular feeding supplementation, either formula, water, or solids, and the use of a pacifier (which reduce the total suckling stimulus directed to the nipple) should be avoided. (Figure 1)
Is LAM Effective?
Many studies have demonstrated that LAM is about 98% effective in the first six months, as long as the following criteria are met:
- The mother is fully or almost fully breastfeeding both day and night with no long intervals between feeds
- No periods have recommenced
- Baby is not receiving any other type of supplement feeds
